Kindly arrive 15 minutes before your scheduled appointment to allow time for check-in and any required paperwork.
What to Bring
- A valid photo ID
- Your insurance card(s) (primary and secondary, if applicable)
- Worker’s compensation or personal injury claim information, if relevant
- Payment for your co-pay (credit and debit cards accepted)
- A complete list of your current medications, or the medications themselves
